What are my dental benefits?

1. Preventive care is covered at 100 percent.
2. You pay a $50 individual and $150 family annual deductible before the Dental Plan starts paying benefits (except for eligible preventive care services).
3. Once you meet the annual deductible, your care is covered when you use Legacy + Network providers:
• Basic* services are covered at 80 percent.
• Major* services are covered at 60 percent.
• Orthodontia is covered at 50 percent up to $3,000 per person. A one-year waiting period applies.

How do I use my dental benefits?

Taking advantage of your dental benefits is easy:

1. Check with Moda Health, Legacy’s third-party dental plan administrator, to be sure your dentist is in network. You can use their online search here, or reach them by phone at 503-243-3886.
2. Make an appointment with a dentist and show your Moda Health ID card before receiving service.
3. You will be billed for services not covered by your plan.

Am I getting a good deal on dental coverage through Legacy?

Legacy’s orthodontia benefit is especially strong compared to similar health care organizations, and coverage for major services (such as crowns, bridges, and dentures) is also better than average. Legacy charges slightly less for employee-only coverage each month, while dental deductibles are on par with similar organizations.
