What are my vision benefits?

  1. Exams are covered at 100 percent after a $15 co-pay.
  2. There is a $200 allowance for frame available every 2 calendar years in the base plan, or every calendar year in the buy-up option. You receive 20 percent off the remaining balance.
  3. For lenses for glasses, the Vision Plan pays 100 percent after a $0–$40 co-pay (depending on lens type). This is in lieu of contact lenses.
  4. For contacts, there is a $200 allowance every calendar year (in lieu of lenses for glasses). You receive 20 percent off the remaining balance.

How do I use my vision benefits?

First, check to see if your provider is in network. Our third-party vision plan administrator, EyeMed, always has current in-network provider information. You can see a complete list of providers within the Legacy + Network online here by searching the “Access” network, or call EyeMed at 866-939-3633 to receive assistance over the phone. (Note that the Legacy Devers Eye Institute is in your network even though you may not see it listed in the online EyeMed provider directory.) Show your EyeMed ID before you receive vision care.

Am I getting a good deal on vision coverage through Legacy?

Legacy’s Vision Plan allows you to choose the coverage level that fits your needs. The base plan offers benefits equivalent to plans at comparable organizations, and the buy-up option offers stronger benefits than most other plans in the industry. Benefits for frames, lenses, and contacts are stronger at Legacy even though our vision premiums are comparable to those at other health care organizations.
